Supportive Services

We offer a variety of services to support your mental and physical well-being, including Care Management, Peer Support, and Community Teams, ensuring you always have the help you need.

Supportive Services That Empower Your Health

Tailored Care Management (TCM)

Care Management supports your overall health by having a single Care Manger who work with you and your team to identify needs that affect your physical and behavioral health. Together, you and your team will develop a Plan of Care to address those needs. This could include housing, food, transportation, and other basic needs, along with medical and mental health needs. Your care manager will assess your needs and strengths and look for ways to better your health and work towards your goals.

Peer Support Services (PSS)

Peer Support Services is a community-based, recovery-oriented service for adults with identified mental health and/or substance abuse issues. PSS differs from other services in that individuals in need are paired with peers who have lived experience, trained, and certified individuals who are stable in recovery from their own mental health or substance abuse issues, and who can listen, provide guidance, and instill hope for recovery based on their own personal experiences. Peer Support allows those seeking recovery the opportunity to learn directly from those who have been, and continue, walk that same path.

Non-Emergency Medical Transportation (NEMT)

Non-Emergency Medical Transportation (NEMT) is a Medicaid service for eligible individuals who need to get to and from medical services when the need does not involve an immediate threat to the life or health, and the individual does not have means of transportation. Having no means of transportation may include not having a working car, not having a driver’s license, having a physical or mental disability, or being unable to travel or wait for a ride alone. B & D Integrated Health Services works with individuals to verify whether a participant is eligible for NEMT services prior to requesting or scheduling a trip.

Transition Management Services (TMS)

Transition Management Services (TMS) is a service provided to individuals participating in the Transition to Community Living Initiative (TCLI). Participation in TMS is assigned by the MCO/LME for individuals with TCLI housing slots. TMS is a rehabilitation service intended to increase and restore an individual’s ability to live successfully in the community.

B & D Integrated Health Services is committed to ensuring that clients receive health services regardless of their ability to pay. To achieve this, we utilize the Sliding Fee Discount Program. As a nonprofit organization serving the community, this program is designed to remove financial barriers to care. Eligibility for the Sliding Fee Discount Program is determined by Federal Poverty Guidelines, taking into consideration household size and annual income. Dependent upon household income verification, fees for services range from $25 to $160 per visit, and both insured and uninsured individuals may apply. Additionally, due to NC DHHS CCBHC grant funding, most behavioral health services are provided at no cost for individuals with a household income at or below 300% of the Federal Poverty Level. Our commitment to accessibility ensures that essential health services are available to all who need them.
